The health and well-being of your student is a priority at Burris Laboratory School. A full-time school nurse is on duty daily to support student health needs, manage medical forms and immunizations, and help families navigate school health requirements.
Required Physicals
All entering kindergarten, fourth, and ninth-grade students must have a current school physical completed by a physician on file with the nurse. If your child is new to Burris and not in one of these grades, an updated physical is still required.
Sports Physicals (Athletes)
Students participating in interscholastic sports must complete the IHSAA Sports Physical Form and turn it in to the athletic director — this form is separate from the school physical form.
Medication Authorization
If your student needs to take a prescribed or over-the-counter medication during school hours, a medication authorization form must be completed and returned to the health clinic.
